Please ensure you have registered for all of your sports, HERE. All athletes, and Unified partners will need to register for each sport they are participating in, each season.

Resources for completing the Athlete Application

The Athlete Application is required for participation as a Special Olympics Colorado athlete! Athletes should complete the online application below, which also requires you to upload a Physical Examination. Use the resources below to help you complete the application!

Steps to Complete the ONLINE Athlete Application:

STEP 1
Have a licensed Medical Professional qualified to conduct exams & prescribe medications fill out the Physical Examination Form or a Sports Physical that clears you for athletic participation.

STEP 2
Go to the athlete application and upload the completed Sports Physical Examination and continue answering the questions.
  • Check out the video to the right for information on how to upload your sports physical or physical examination form into SOCO’s Online Medical Application.
  • You will receive an email with your responses when the application is submitted. If you do not receive an email (check Spam/Junk) contact the Help Desk.
  • Applicants with incomplete information will be contacted within 2-3 business days. Your application will not be considered valid until resolved.

Frequently Asked Questions – New SOCO Athlete Application (2025)

What is Changing?
  • A doctor’s exam is no longer required as part of the Athlete Application.
  • Athletes must renew their application annually (instead of every 3 years) with a short renewal application.
Why is There a New Athlete Application?

This change aligns with Special Olympics International’s effort to make participation easier while still gathering the essential health information coaches need for safe training and competition.

Who Must Complete the New Athlete Application?
  • New athletes joining a Special Olympics Colorado team
    • A parent/guardian must complete the Athlete Application for minors and for adults with legal guardians.
  • School-based athletes participating in a SOCO-hosted competition
  • Current athletes whose existing application (valid for three years) is expiring
