If you already completed the exam form and athlete application in 2023 or 2024, Click Here to register for the upcoming sport season.

Please ensure you have registered for all of your sports, HERE. All athletes, and Unified partners will need to register for each sport they are participating in, each season.

The Athlete Application is required for participation as a Special Olympics Colorado athlete! Athletes should complete the online application below, which also requires you to upload a Physical Examination. Use the resources below to help you complete the application!

Have a licensed Medical Professional qualified to conduct exams & prescribe medications fill out the Physical Examination Form or a Sports Physical that clears you for athletic participation.

Go to the athlete application and upload the completed Sports Physical Examination and continue answering the questions.

You will receive an email with your responses when the application is submitted. If you do not receive an email (check Spam/Junk) contact the Help Desk.
Applicants with incomplete information will be contacted within 2-3 business days. Your application will not be considered valid until resolved.
